    Background:

    • Intra-abdominal testes are associated with an increased risk of malignant transformation.
    • Testicular tumors in undescended testes require careful consideration for diagnosis and management.

    Observation:

    • A rare case of malignant change in an intra-abdominal testis is presented in a Black male.
    • The tumor exhibited a mixed cellular picture histologically, an uncommon feature for testicular tumors.
    • Serum tumor markers were absent despite the presence of seminoma, and local invasive spread was noted.

    Findings:

    • The combination of mixed histology, absent tumor markers with seminoma, and local invasion makes this a unique presentation.
    • Orchiopexy (surgical repositioning) does not mitigate the risk of malignancy in undescended testes.
    • Orchiopexy does not significantly improve spermatogenesis in postpubertal patients.

    Implications:

    • Orchiectomy (surgical removal) is advised for all maldescended testes to manage cancer risk.
    • Orchiectomy should be considered mandatory for intra-abdominal testes due to the high risk of malignancy.
    • This case highlights the importance of vigilance for testicular cancer in undescended testes, regardless of tumor marker status.
