    Summary

    Conventional antihistamines effectively treat chronic idiopathic urticaria in most patients. A small subset may benefit from adding H2 blockers if initial treatment with H1 blockers is insufficient.

    Background:

    • Chronic idiopathic urticaria (CIU) is a common condition.
    • Conventional antihistamines are the first-line treatment for CIU.

    Purpose of the Study:

    • To evaluate the efficacy of conventional antihistamines in treating CIU.
    • To determine the role of H2 blockers in refractory CIU cases.

    Main Methods:

    • Two hundred unselected patients with CIU were treated with adequate doses of conventional antihistamines.
    • Patients with incomplete response received additional H2 blockers (cimetidine) or increased H1 blocker doses.

    Main Results:

    • 98.5% of patients achieved complete disease remission with conventional antihistamines.
    • Three patients had incomplete response; two cleared with cimetidine, one with increased H1 blocker dose.

    Conclusions:

    • Adequate doses of conventional antihistamines are highly effective for most CIU patients.
    • H2 blockers are beneficial for a small percentage of CIU patients unresponsive to H1 blockers.
