    Background:

    • Hyperparathyroidism is a condition involving excess parathyroid hormone.
    • Ocular manifestations of hyperparathyroidism are rare.
    • Scleral calcifications are not a commonly recognized sign.

    Observation:

    • Two patients with hyperparathyroidism and normal eyes showed scleral calcifications on imaging.
    • Computed tomography (CT) revealed calcification in one patient, while plain films also showed it.
    • CT uniquely identified a solitary scleral plaque in the second patient.

    Findings:

    • Histopathology confirmed calcium plaques in the posterior sclera of a third hyperparathyroid patient.
    • The observed calcifications correlate with CT findings.
