    Background:

    • Leg ulcers represent a significant clinical challenge with limited effective treatments.
    • Human amniotic membrane has shown promise in surgical applications, but the underlying mechanisms are not fully understood.

    Purpose of the Study:

    • To investigate the effects of cultured human amnion on the cellular and tissue characteristics of leg ulcer beds.
    • To identify potential angiogenic factors within human amnion contributing to wound healing.

    Main Methods:

    • Biopsy specimens were collected from fifteen leg ulcer patients before and after a 5-day application of cultured human amnion.
    • Histological analysis examined changes in granulation tissue, connective tissue, vascularity, basement membranes, and endothelial cells (including factor VIII expression).

    Main Results:

    • Significant granulation tissue formation was observed in ulcer beds post-amnion application.
    • Microscopic examination revealed thinned connective tissues, enhanced vessel development, and more compact vascular basement membranes.
    • Increased presence of factor VIII granules within endothelial cells indicated stimulated angiogenesis.

    Conclusions:

    • Cultured human amnion contains angiogenic factors that promote significant tissue regeneration in leg ulcers.
    • These findings provide a scientific basis for the clinical efficacy of amniotic membranes in surgical wound management.
