    Background:

    • Otitis media with effusion (OME) is a common condition characterized by fluid in the middle ear.
    • The cellular composition of middle ear effusions and its relation to mucosal damage is not fully understood.

    Purpose of the Study:

    • To investigate the cellular immune response in otitis media with effusion.
    • To determine the types and proportions of immune cells present in middle ear effusions.
    • To explore the potential role of these cells in the pathogenesis of OME and associated mucosal damage.

    Main Methods:

    • Analysis of cellular populations in middle ear effusions from OME patients.
    • Identification of lymphocyte subsets using alpha-naphthyl-acetate-esterase (ANAE) staining.
    • Confirmation of T-lymphocyte presence and numbers using E-rosette assays.

    Main Results:

    • Granulocytic cells predominated (60-70%) in OME effusions.
    • Monocytic-lymphocytic cells constituted the major cell type in 20% of cases.
    • A significant proportion (15-20%) of effusions showed sparse cellularity.
    • T-lymphocytes, identified by ANAE, were abundant in effusions, with numbers comparable to peripheral blood.
    • E-rosette tests corroborated ANAE findings for T-lymphocytes.

    Conclusions:

    • The cellular profile of OME effusions is predominantly granulocytic.
    • Normal T-lymphocyte counts in effusions suggest a potential link to delayed-type hypersensitivity reactions in mucosal damage.
    • Immune complexes may also contribute to mucosal damage in sterile middle ear effusions.
