    Background:

    • Dermatomyositis is an idiopathic inflammatory myopathy.
    • Corticosteroids are the first-line treatment but can be unpredictable and cause side effects.
    • Severe cases may be refractory to standard therapies.

    Purpose of the Study:

    • To evaluate the efficacy of intravenous methotrexate in patients with severe, corticosteroid-refractory dermatomyositis.
    • To assess the long-term outcomes of methotrexate treatment in this patient population.

    Main Methods:

    • Two patients with severe dermatomyositis unresponsive to corticosteroids received intravenous methotrexate.
    • Treatment response was monitored over a 2-year follow-up period.
    • Muscle enzyme levels were assessed to evaluate disease activity.

    • Both patients showed satisfactory and sustained improvement with methotrexate therapy.
    • Muscle enzyme studies indicated resolution of active muscle inflammation.
