    Summary

Idiopathic cholesterol pneumonitis (ICP) shows a male predominance and often affects a single lobe. This rare lung disease may mimic lung cancer and could stem from surfactant metabolism issues.

    Area of Science:

    • Pulmonology
    • Pathology
    • Radiology

    Background:

    • Idiopathic cholesterol pneumonitis (ICP) is a rare lung disease characterized by lipid-laden macrophages.
    • Reviewing the 50th reported case and 49 prior cases provides a comprehensive overview of ICP.

    Observation:

    • ICP exhibits a marked male predominance and frequently involves a single lung lobe.
    • Lesion distribution, dense pleural fibrosis, and a history of smoking are notable features.
    • Histologically, giant cells are closely associated with disrupted alveolar wall elastic fibers.

    Findings:

    • ICP shares clinical and radiological similarities with primary bronchogenic carcinoma.
    • The condition's etiology is debated, with theories exploring surfactant metabolism.
    • A parallel is drawn between ICP and spontaneous lipid pneumonia in mongooses.

    Implications:

    • Understanding ICP's features aids in differentiating it from lung cancer.
    • Further research into surfactant metabolism may elucidate the pathogenesis of ICP.
    • This review highlights key diagnostic and etiological aspects of idiopathic cholesterol pneumonitis.
