    Summary

    Perineural invasion, common in normal or hyperplastic prostates, is not solely indicative of cancer. This finding challenges the long-held belief that perineural invasion definitively proves malignancy.

    Background:

    • Perineural invasion (PNI) is often considered a hallmark of prostate cancer malignancy.
    • The presence of PNI in prostate tissue has historically been interpreted as definitive evidence of cancer.

    Purpose of the Study:

    • To investigate the occurrence and significance of perineural invasion in non-cancerous prostate tissues.
    • To re-evaluate the diagnostic value of perineural invasion in prostate pathology.

    Main Methods:

    • Histological examination of serial prostate sections from autopsies.
    • Analysis of 8 consecutive autopsies, including normal and hyperplastic prostate tissue.
    • Identification and characterization of perineural invasion foci and associated structures.

    Main Results:

    • Perineural invasion was identified in 7 out of 8 autopsies (87.5%) in normal or hyperplastic prostate tissue.
    • A close association between nerves, glandular epithelium, and vascular structures was observed in all 8 cases.
    • Differences in epithelial characteristics were noted between PNI in carcinoma and in normal/hyperplastic prostates.

    Conclusions:

    • Perineural invasion occurs frequently in non-malignant prostate conditions, including normal and hyperplastic states.
    • PNI can no longer be considered unequivocal evidence of prostate malignancy.
    • The interpretation of perineural invasion requires careful consideration of histological context and epithelial morphology.
