    Summary

    Cough syncope linked to carotid sinus hypersensitivity can be treated by denerving the hypersensitive sinus. This case highlights the need to monitor both blood pressure and heart rate in suspected carotid sinus syndrome.

    Background:

    • Cough syncope is a form of reflex syncope.
    • Carotid sinus hypersensitivity (CSH) can cause syncope.
    • CSH can present with cardioinhibitory, vasodepressor, or mixed responses.

    Observation:

    • A patient presented with cough syncope.
    • The patient exhibited CSH with mixed cardioinhibitory and vasodepressor responses.
    • Spontaneous atypical Wenckebach cycles were observed.

    Findings:

    • Surgical denervation of the more hypersensitive carotid sinus ameliorated the patient's symptoms.
    • The observed Wenckebach cycles were attributed to the combined hypersensitive cardioinhibitory and vasodepressor responses.
