    Background:

    • Junctional premature beats are ectopic beats originating from the atrioventricular junction.

    • Coupling intervals less than 425 milliseconds (msec) resulted in concealed beats, while intervals greater than 425 msec led to manifest beats.
    • Persistent long runs of pseudo 2:1 AV conduction were observed.
    • Extrasystolic beats exhibited exit block, and interectopic sinus beats followed the formula 2n-1.

    Implications:

    • Understanding these electrocardiographic patterns is crucial for accurate diagnosis and differentiation from true AV block.
    • This case highlights the complex electrophysiological behavior of premature junctional beats and their impact on cardiac rhythm.
