    Circulating anticoagulants (CA) are linked to biological false-positive syphilis tests (BFP-STS) in autoimmune patients. This association was not found in syphilis patients or the elderly, suggesting specific disease-related mechanisms.

    Background:

    • Circulating anticoagulants (CA), including lupus anticoagulant, are often associated with biological false-positive tests for syphilis (BFP-STS).
    • This association is frequently observed in patients diagnosed with collagen diseases.

    Purpose of the Study:

    • To investigate the prevalence of CA in patients with chronic BFP-STS without collagen or autoimmune diseases.
    • To determine if CA is present in patients with syphilis or elderly individuals with BFP-STS.

    Main Methods:

    • Sera from 30 subjects with chronic BFP-STS (without collagen/autoimmune diseases), 21 syphilis patients, and 21 elderly subjects with BFP-STS were tested for CA.
    • Standard serological and immunological assays were employed.

    • CA was detected in 8 out of 30 (26.7%) subjects with chronic BFP-STS but no collagen or autoimmune diseases.

    • Differences in autoantibody behavior likely explain the varying incidence of CA-BFP-STS association across different patient groups.
