Beta 2-microglobulin and other proteins as parameter for tubular function.

M Weise, D Prüfer, G Jaques

    Contributions to Nephrology

    Summary

Urinary biomarkers like beta 2-microglobulin detect kidney damage from preeclampsia and transplant rejection. Certain antibiotics increase these markers, indicating potential tubular injury.

    Area of Science:

    • Nephrology
    • Clinical Chemistry
    • Biomarker Discovery

    Background:

    • Urinary low molecular weight proteins and enzymes are sensitive indicators of proximal tubular damage.
    • Preeclampsia and kidney transplantation involve renal dysfunction where these markers can be altered.

    Purpose of the Study:

    • To evaluate the utility of urinary beta 2-microglobulin, alanine aminopeptidase (AAP), gamma-glutamyl transpeptidase (gamma-GT), and alkaline phosphatase (AP) in diagnosing kidney conditions.
    • To differentiate between primary preeclampsia and superimposed chronic pyelonephritis.
    • To monitor kidney transplant rejection and assess drug-induced nephrotoxicity.

    Main Methods:

    • Measurement of urinary beta 2-microglobulin, AAP, gamma-GT, and AP levels.
    • Analysis of marker excretion in patients with preeclampsia, kidney transplants, and healthy individuals receiving specific medications.

    Related Experiment Videos

    Main Results:

    • Urinary beta 2-microglobulin levels help distinguish primary preeclampsia (3-4 fold increase) from preeclampsia superimposed on chronic pyelonephritis (up to 300 fold increase).
    • Kidney transplant recipients show elevated levels of beta 2-microglobulin, AAP, gamma-GT, and AP, with further increases preceding clinical signs of rejection in over 80% of cases.
    • Therapeutic doses of analgesics (paracetamol, aspirin) did not affect tubular markers, while aminoglycosides caused cumulative increases in beta 2-microglobulin and AAP, and cephalosporins increased total urinary proteins.

    Conclusions:

    • Urinary beta 2-microglobulin and tubular enzymes are valuable biomarkers for proximal tubular injury in various clinical settings.
    • These markers aid in differentiating preeclampsia subtypes and detecting early kidney transplant rejection.
    • Antibiotic-induced nephrotoxicity can be monitored through changes in these urinary proteins.
