    Background:

    • Digoxin is a cardiac glycoside used to treat heart failure.
    • Digoxin toxicity can cause life-threatening cardiac arrhythmias.
    • Anti-digoxin antibody fragments are used as a specific antidote.

    Purpose of the Study:

    • To investigate the mechanism of action of anti-digoxin antibody in reversing digoxin intoxication.
    • To quantify digoxin levels in plasma, urine, and myocardial tissue after antibody administration.

    Main Methods:

    • Sheep were immunized to produce anti-digoxin antibody (whole serum or gamma globulin).
    • Six dogs received digoxin intravenously, followed by antibody treatment.
    • Plasma, urine, and myocardial digoxin levels (bound and free) were measured serially.
    • A control group of six dogs received digoxin without antibody.

    Main Results:

    • Anti-digoxin antibody significantly reduced digoxin levels in myocardial tissue.
    • Antibody administration led to increased plasma digoxin levels, indicating redistribution.
    • Urinary digoxin excretion did not correlate with toxicity reversal.

    Conclusions:

    • Anti-digoxin antibody acts by inactivating digoxin within the myocardium and facilitating its removal into the plasma.
    • This redistribution mechanism is the primary driver of toxicity reversal, not enhanced urinary excretion.
