    Background:

    • Total joint transfers are a reconstructive option for hand injuries.

    Purpose of the Study:

    • To compare the outcomes of autogenous vascularized and nonvascularized toe proximal interphalangeal joint transfers in a primate model.
    • To evaluate the clinical and histopathological viability of these grafts for hand reconstruction.

    Main Methods:

    • Nine Macaca fascicularis monkeys underwent toe-to-hand proximal interphalangeal joint transfers.
    • Grafts were assessed clinically and histopathologically over 16 weeks to 10 months.
    • A skin island was incorporated as a viability monitor, and the extensor mechanism was included.

    Main Results:

    • Seven of nine vascularized grafts survived with preserved hyaline articular surfaces.
    • Four nonvascularized grafts survived but showed cartilage necrosis and degeneration.
    • Complications included infection and graft failure in both groups, but were more frequent in nonvascularized grafts.

    Conclusions:

    • Autogenous vascularized toe joint transfers are a viable option for hand reconstruction, offering better preservation of articular cartilage.
    • The described technique, including a viability monitor and extensor mechanism, is suitable for clinical application.
