    • A healthy 50-year-old male presented with bilateral retinal pigment epithelium (RPE) mottling in the macula.
    • The left eye was enucleated due to choroidal melanoma.

    • Histopathology revealed a thickened sclera with abnormal collagen fibrils (up to 5,800 A) and a compressed choroid.
    • The overlying RPE showed hypo/hyperpigmentation and hyperplasia, with intracytoplasmic lipofuscin.

    Implications:

    • This report establishes a new clinicopathologic entity.
    • Further research is needed to understand the pathogenesis and clinical significance of this maculopathy.
