    Background:

    • Refractory acute leukemia presents a significant therapeutic challenge.
    • Limited effective treatment options exist for patients with relapsed or refractory acute myeloid leukemia (AML).

    Purpose of the Study:

    • To evaluate the efficacy and safety of 5-Azacytidine in patients with refractory acute leukemia.
    • To determine the response rate and necessary treatment courses for achieving remission.

    Main Methods:

    • 11 patients with refractory acute leukemia received 5-Azacytidine (150 mg/m2/day) via continuous infusion for 5 days every 2 weeks.
    • Patients had prior treatment with anthracycline/cytosine arabinoside protocols.
    • Response was assessed in patients with refractory de novo acute myelogenous leukemia.

    Main Results:

    • 6 out of 8 patients (75% response rate) with refractory de novo AML achieved remission (3 complete, 3 partial).
    • An average of 1.67 courses were needed for response.
    • No remissions were observed in blastic chronic myelogenous leukemia or secondary acute leukemia.
    • Common toxicities included myelosuppression, nausea, vomiting, abnormal liver function, and neuromuscular symptoms.

    Conclusions:

    • Continuous infusion 5-Azacytidine exhibits significant activity in refractory acute myelogenous leukemia.
    • This regimen should be considered for primary remission induction therapy in AML.
    • Further investigation into its role in AML treatment protocols is warranted.
