    Summary

    Cough syncope, a loss of consciousness after coughing, is usually due to reduced blood flow to the brain. This case reveals a new cause: temporary heart block during coughing fits.

    Background:

    • Its pathophysiology is linked to increased intrathoracic pressure, reduced cardiac output, and impaired cerebral venous return via a Valsalva mechanism.

    • A case of cough-induced syncope was investigated using electrocardiogram (ECG) monitoring.
    • The monitoring revealed a previously undocumented cardiac event during coughing episodes.

    Findings:

    • Coughing triggered paroxysmal atrioventricular block.
    • This was followed by ventricular asystole, leading to syncope.
