Polyclonal and antigen-specific B-cell responses in patients with common variable immunodeficiency.

Summary

Patients with common variable immunodeficiency (CVID) show varied B-cell antibody production. Many CVID patients have functional B cells, but antigen-specific responses differ from polyclonal activation, suggesting distinct immune pathways.

Background:

Common variable immunodeficiency (CVID) is characterized by hypogammaglobulinemia.
Investigating B-cell function is crucial for understanding CVID pathogenesis.

Purpose of the Study:

To investigate antigen-specific antibody responses in patients with CVID.
To explore the capacity of B cells from CVID patients to produce antibodies in vitro.

Main Methods:

Peripheral blood lymphocyte cultures from 32 CVID patients were sensitized in vitro with sheep red blood cell determinants.
Antibody-secreting cells were quantified using a hemolytic plaque assay.
T-cell help was amplified using concanavalin A or allogeneic T cells.

Four patient groups (A-D) were identified based on response levels.

Group A (10 patients) showed robust antibody secretion.

Groups B (9 patients) and C (7 patients) showed suboptimal or T-cell-dependent responses, respectively.

Group D (7 patients) showed no response.

Only 10 responders (Groups A & B) showed polyclonal immunoglobulin secretion.

Conclusions:

The majority of CVID patients possess B cells capable of in vitro antibody production.
Differential responses in antigen-specific versus polyclonal assays suggest distinct B-cell activation pathways in CVID.
