    Background:

    • Diabetic retinopathy is a leading cause of vision loss.
    • Rhegmatogenous retinal detachment (RRD) in diabetic patients presents unique surgical challenges.

    Purpose of the Study:

    • To evaluate the surgical outcomes of vitrectomy for diabetic RRD.
    • To identify factors influencing visual acuity prognosis after surgery.

    Main Methods:

    • Retrospective analysis of 107 eyes undergoing vitrectomy for diabetic RRD.
    • Assessment of surgical success, visual acuity, and preoperative prognostic factors.

    Main Results:

    • Successful treatment of the rhegmatogenous component in 74% of eyes.
    • Persistent macular traction detachment occurred in 14% of successfully treated eyes.
    • Final visual acuity improved in 53%, worsened in 41%, and remained unchanged in 6%.

    Conclusions:

    • Vitrectomy is effective for diabetic RRD, but visual outcomes are variable.
    • Preoperative iris neovascularization and macular detachment are poor prognostic indicators for visual acuity.
