    Summary

    This is the first reported case of persistent atrial standstill linked to limb girdle muscular dystrophy. Despite severe myopathy, the patient successfully delivered a healthy child, suggesting preserved cardiac function during pregnancy.

    Background:

    • Persistent atrial standstill is a rare cardiac arrhythmia.
    • Limb girdle muscular dystrophy is a group of inherited muscle-weakening diseases.
    • The association between these two conditions has not been previously documented.

    Observation:

    • A pregnant patient with advanced limb girdle muscular dystrophy presented with persistent atrial standstill.
    • The patient underwent hemodynamic studies, including coronary cinearteriography, postpartum.
    • Clinical and radiological findings suggested a peptic ulcer, potentially due to excessive vagal tone.

    Findings:

    • The patient delivered an apparently normal child despite the severe myopathy and cardiac condition.
    • Hemodynamic assessments, including coronary cinearteriography, were normal in the postpartum period.
    • A congenital anomaly of the conduction system was hypothesized to explain the co-occurrence of left anterior hemiblock and atrial standstill but could not be confirmed.

    Implications:

    • This case highlights the possibility of successful pregnancy outcomes in patients with limb girdle muscular dystrophy and persistent atrial standstill.
    • The findings suggest that advanced myopathy does not necessarily preclude a normal delivery.
    • Further research is needed to elucidate the underlying mechanisms connecting limb girdle muscular dystrophy, persistent atrial standstill, and potential autonomic dysfunction.
