    Summary

    Serum beta 2-microglobulin (S-beta 2m) is a valuable prognostic marker in malignant lymphoma. Elevated S-beta 2m levels at diagnosis indicate a poorer prognosis and higher relapse rates in both Hodgkin's disease and non-Hodgkin's lymphoma patients.

    Background:

    • Serum beta 2-microglobulin (S-beta 2m) is a protein found on the surface of most nucleated cells.
    • Its levels can be elevated in various conditions, including malignancies.

    Purpose of the Study:

    • To investigate the prognostic significance of serum beta 2-microglobulin (S-beta 2m) levels at diagnosis in patients with malignant lymphoma.
    • To correlate S-beta 2m levels with disease stage, prognosis, and treatment response.

    Main Methods:

    • Serum beta 2-microglobulin (S-beta 2m) was measured at diagnosis in 189 patients with malignant lymphoma (149 non-Hodgkin's lymphoma, 40 Hodgkin's disease).
    • Patients had normal serum creatinine clearance.
    • S-beta 2m levels were correlated with disease stage, relapse rates, survival, and remission status.

    Main Results:

    • Elevated S-beta 2m (>3.0 mg/l) was more frequent in advanced stages (III/IV) compared to early stages (I/II) for both NHL (65% vs. 15%) and HD (83% vs. 11%).
    • High pretreatment S-beta 2m levels were associated with poor prognosis in all stages of NHL.
    • Patients with Stage I/II NHL and elevated S-beta 2m had higher relapse rates.
    • Advanced stage patients with S-beta 2m >3.5 mg/l had significantly shorter survival.
    • Achieved complete remission correlated with lower S-beta 2m levels in Stage III/IV NHL patients.

    Conclusions:

    • Serum beta 2-microglobulin (S-beta 2m) is a valuable prognostic indicator in malignant lymphoma.
    • Monitoring S-beta 2m levels can aid in predicting treatment outcomes and identifying patients at higher risk of relapse.
