    Background:

    • Elevated plasma digoxin concentrations observed during verapamil treatment.
    • This interaction is partly attributed to reduced renal clearance of digoxin.
    • Inhibition of the digoxin tubular secretory process is a suspected mechanism.

    Purpose of the Study:

    • To investigate the in vitro effect of verapamil on digoxin uptake in various rat tissues.
    • To elucidate the role of verapamil in digoxin-verapamil interaction.
    • To determine if verapamil alters digoxin distribution in cardiac and muscle tissues.

    Main Methods:

    • In vitro study using rat tissue slices.
    • Incubation of tissue slices with [125I]digoxin.
    • Addition of verapamil to the incubation medium to assess its impact on digoxin uptake.
    • Measurement of slice/medium digoxin ratios.

    Main Results:

    • Verapamil significantly reduced digoxin uptake in kidney tissue by 15.6% (p < 0.0005).
    • Digoxin uptake in liver, heart, and striated muscle tissues was not affected by verapamil.
    • Kidney tissue showed the highest slice/medium digoxin ratio (4.09 +/- 0.60).

    Conclusions:

    • Verapamil may inhibit the renal cortical transport mechanism of digoxin.
    • The observed increase in serum digoxin concentration during verapamil treatment is likely due to reduced renal clearance.
    • Verapamil does not appear to cause digoxin redistribution from the heart or striated muscle.
