Background:

Sarcoidosis is an inflammatory disease characterized by granuloma formation.
The exact pathogenesis of sarcoidosis, particularly skin manifestations, remains incompletely understood.
Immune complex deposition is a proposed mechanism in various autoimmune and inflammatory conditions.

Purpose of the Study:

To investigate the presence and location of immune complex deposits in skin lesions of patients with active sarcoidosis.
To determine if immune deposits are also present in clinically normal skin of these patients.
To evaluate the potential role of immune complex deposition in sarcoidosis pathogenesis.

Main Methods:

Immunofluorescence microscopy was used to examine biopsy specimens.
Samples were obtained from granulomatous skin lesions of 14 patients with active sarcoidosis.
Biopsy specimens from clinically normal skin of the same patients were also analyzed.

Main Results:

Deposits of Immunoglobulin M (IgM), Immunoglobulin A (IgA), or complement C3 were identified in the dermal vessel walls and/or dermal-epidermal junction in seven of the fourteen granulomatous lesions.
Similar deposits were observed in three of the twelve biopsy specimens from clinically normal skin.
These findings suggest a potential link between immune complex deposition and sarcoidosis skin lesions.

Conclusions:

The presence of immunoglobulin and complement deposits in sarcoidosis skin lesions supports the immune complex hypothesis.
Immune complex deposition in dermal vessel walls may play a role in the development of granulomatous skin lesions in sarcoidosis.
Further research is warranted to elucidate the precise mechanisms and implications of immune complex deposition in sarcoidosis.
