Platelet aggregation following heparin and protamine administration.

N Ellison, L H Edmunds, R W Colman

    Anesthesiology

    Summary

Heparin enhances platelet aggregation sensitivity to ADP and epinephrine. Protamine reverses this effect, reducing sensitivity, especially when neutralizing heparin, without affecting aggregation alone.

    Area of Science:

    • Hematology
    • Pharmacology

    Background:

    • Platelet aggregation is crucial for hemostasis.
    • Heparin and protamine are commonly used in clinical settings.
    • Understanding their effects on platelet function is important.

    Purpose of the Study:

    • To investigate the in vitro effects of heparin, protamine, and their complex on platelet aggregation.
    • To determine how these substances alter platelet sensitivity to adenosine diphosphate (ADP) and epinephrine.

    Main Methods:

    • Blood from normal volunteers was treated with heparin, protamine, or heparin-protamine complexes.
    • Threshold concentrations of ADP and epinephrine inducing platelet aggregation were measured.
    • Geometric means of threshold concentrations were compared across different treatment groups.

    Related Experiment Videos

    Main Results:

    • Heparin significantly decreased threshold concentrations for ADP and epinephrine, indicating increased platelet sensitivity.
    • Protamine, particularly in neutralizing concentrations, increased threshold concentrations, reducing platelet sensitivity.
    • Protamine alone did not affect platelet aggregation; excess protamine showed no additional effect.

    Conclusions:

    • Heparin potentiates platelet aggregation response to ADP and epinephrine.
    • Protamine counteracts heparin's effect and can reduce platelet sensitivity, especially when used to neutralize heparin.
    • These findings have implications for managing patients on anticoagulant therapy.
