    Background:

    • Arterial occlusive disease (AOD) impairs lower limb circulation.
    • Treadmill exercise is a standard stress test for assessing functional capacity in AOD.
    • Alternative, less demanding stress tests are needed for patient populations unable to perform treadmill exercise.

    Purpose of the Study:

    • To compare the ankle pressure response during reactive hyperemia with the response during treadmill exercise.
    • To evaluate reactive hyperemia as a potential substitute for treadmill testing in patients with AOD.

    Main Methods:

    • Ankle pressure response to reactive hyperemia and treadmill exercise was measured.
    • 28 limbs from 14 normal individuals and 26 legs from 15 patients with AOD were studied.

    Main Results:

    • Normal limbs showed a mean ankle pressure drop of 17% during reactive hyperemia, significantly less than the 54% drop in AOD legs (P < .001).
    • Abnormal reactive hyperemia responses were observed in nearly all AOD limbs.
    • A strong correlation (r = 0.71, P < .001) was found between ankle pressure responses to reactive hyperemia and treadmill exercise.

    Conclusions:

    • Ankle pressure measurement during reactive hyperemia is a valid indicator of circulatory functional capacity during stress in AOD.
    • Reactive hyperemia testing is a time-efficient, equipment-light alternative to treadmill testing.
    • This method is suitable for patients at risk or unable to perform treadmill exercise.
