    Background:

    • Novel pharmacologic agents effectively suppress premature ventricular beats.
    • Chronic efficacy of these agents in sustained ventricular arrhythmias is uncertain.

    Purpose of the Study:

    • To investigate the chronic efficacy and safety of flecainide acetate in patients with sustained ventricular arrhythmias.
    • To assess the potential for drug-induced arrhythmias in a high-risk patient population.

    Main Methods:

    • Five patients with recurrent sustained ventricular arrhythmias were treated with flecainide acetate (200 mg twice daily).
    • Clinical observation focused on the appearance of arrhythmias and drug toxicity.

    Main Results:

    • Three out of five patients developed ventricular tachycardia with a sinusoidal QRS complex.
    • This indicates a potential pro-arrhythmic effect of flecainide acetate in this patient group.

    Conclusions:

    • Early reports on flecainide's efficacy in stable patients may not apply to sicker patients with recurrent sustained ventricular arrhythmias.
    • Life-threatening arrhythmias can manifest as the initial sign of drug toxicity in these high-risk patients.
