    Acute mitral valve obstruction from vegetations causes severe pulmonary edema and sudden death. Early diagnosis via echocardiography and emergency surgery are crucial for survival in these critical endocarditis cases.

    • Many had subtle cardiac auscultation findings.

    Findings:

    • Three patients experienced sudden, unexpected death.
    • Two patients survived after emergency surgery for valve obstruction.
    • Echocardiography is key for diagnosis.

    Implications:

    • Acute mitral valve obstruction requires urgent surgical intervention.
