Neovascular glaucoma. Etiologic considerations.

G C Brown, L E Magargal, A Schachat

    Summary

Neovascular glaucoma (NVG) often stems from retinal venous obstructive disease, diabetic retinopathy, or carotid artery disease. Extensive retinal ischemia typically precedes NVG, especially in patients with underlying conditions like diabetes.

    Area of Science:

    • Ophthalmology
    • Vascular Medicine

    Background:

    • Neovascular glaucoma (NVG) is a severe condition characterized by abnormal blood vessel growth in the iris and anterior chamber.
    • Understanding the primary etiologies and associated systemic conditions is crucial for managing NVG.

    Purpose of the Study:

    • To review the etiologic associations and demographic characteristics of patients with neovascular glaucoma.
    • To identify key risk factors and preceding conditions associated with NVG development.

    Main Methods:

    • Retrospective review of 208 patients diagnosed with neovascular glaucoma.
    • Analysis of primary etiologic associations, systemic comorbidities, and demographic data.

    Main Results:

    • The most common causes of NVG were retinal venous obstructive disease (36.1%), diabetic retinopathy (32.2%), and carotid artery obstructive disease (12.9%).
    • Systemic arterial hypertension (51%) and diabetes mellitus (46%) were prevalent comorbidities.
    • Bilateral NVG was strongly associated with diabetic retinopathy (24 out of 25 cases).
    • Specific gender predilections were observed based on the underlying cause of NVG.
    • Extensive retinal ischemia preceded iris neovascularization in 97% of NVG cases.

    Conclusions:

    • Retinal venous obstructive disease, diabetic retinopathy, and carotid artery disease are primary drivers of NVG.
    • Systemic conditions like hypertension and diabetes are significant risk factors.
    • Patients with NVG and no apparent fundus pathology may have underlying severe carotid artery disease and require further investigation.
