Cyclic ultrastructural abnormalities in human cyclic neutropenia.

R T Parmley, G J Presbury, W C Wang

    The American Journal of Pathology
    |August 1, 1984

    Summary

Cyclic neutropenia causes abnormal neutrophil development, impacting early myeloid cells. This study reveals a cyclic insult affecting promyelocytes, leading to neutrophil destruction.

    Area of Science:

    • Hematology
    • Cell Biology
    • Immunology

    Background:

    • Cyclic neutropenia is a rare disorder characterized by periodic drops in neutrophil counts.
    • Understanding the cellular mechanisms underlying cyclic neutropenia is crucial for developing targeted therapies.

    Purpose of the Study:

    • To investigate the ultrastructural changes in neutrophils during different phases of the neutrophil cycle in patients with cyclic neutropenia.
    • To identify the specific cell types and maturation stages most affected by the cyclic insult.

    Main Methods:

    • Electron microscopy was used to examine neutrophils from two brothers with cyclic neutropenia.
    • Samples were collected at various time points throughout the 15-21 day neutrophil cycle.

    Main Results:

    • Abnormalities, including increased autophagy and granule defects, were observed in promyelocytes during the early neutropenic phase.
    • During the late neutropenic phase, segmented neutrophils showed increased vacuoles and reduced granules, with significant neutrophil debris in marrow macrophages.
    • These morphologic changes were specific to neutrophils and not observed in other myeloid cells.

    Conclusions:

    • The study demonstrates a cyclic insult in cyclic neutropenia that initially impacts promyelocytes.
    • Dysgranulopoiesis and intramedullary destruction of abnormal myeloid cells contribute to the neutropenia observed in these patients.
