[Course and prognosis in sudden deafness].

T Eichhorn, G Martin

    Summary

Sudden deafness hearing can gradually worsen over 15 years, but spontaneous recovery is common. Treatment effectiveness for sudden deafness requires critical evaluation due to high remission rates.

    Area of Science:

    • Otolaryngology
    • Audiology
    • Neurology

    Background:

    • Sudden deafness is an otologic emergency with variable prognosis.
    • Understanding long-term hearing development is crucial for patient management.

    Purpose of the Study:

    • To investigate the long-term audiometric changes and recovery patterns in patients with sudden deafness.
    • To evaluate the influence of various factors and treatments on the long-term outcome of sudden deafness.

    Main Methods:

    • Follow-up audiograms were conducted up to 15 years post-sudden deafness in 69 patients.
    • Analysis included average pure tone threshold changes, spontaneous recovery rates, and relapse incidence.
    • Correlations between immediate treatment response, patient characteristics, and long-term hearing development were assessed.

    Main Results:

    • Average pure tone threshold declined linearly by approximately 1.15 dB/year.
    • Hearing improvement occurred in 21% of patients up to 12 years post-onset.
    • Spontaneous recovery was observed in 28.6% without initial treatment improvement; 8.3% experienced relapse.

    Conclusions:

    • Long-term hearing in sudden deafness patients shows a gradual decline, but spontaneous recovery is frequent.
    • Immediate treatment response, comorbidities, or delayed treatment did not predict long-term hearing outcomes.
    • The efficacy of current sudden deafness treatments needs critical reassessment given the high spontaneous remission rate.
