    Background:

    • Alcoholic liver disease (ALD) is a major cause of liver damage.
    • Alcoholic hyalin (AH) is a hallmark of ALD, but its precise role in hepatocyte injury is not fully understood.

    Purpose of the Study:

    • To investigate the morphologic and metabolic changes in hepatocytes containing AH.
    • To elucidate the mechanism of hepatocyte injury and death in ALD.

    Main Methods:

    • Analysis of liver biopsies from 14 patients with ALD.
    • Morphologic examination of hepatocytes and their organelles.
    • Assessment of enzyme activities, including glucose-6-phosphatase, succinic dehydrogenase, and diphosphopyridine nucleotide diaphorase.

    Main Results:

    • Hepatocytes with AH showed hydropic swelling, mitochondrial abnormalities, and decreased smooth endoplasmic reticulum.
    • Significant reduction in glucose-6-phosphatase activity was observed.
    • Neutrophils infiltrated hepatocytes, causing cell membrane lysis via lysosomal release.

    Conclusions:

    • AH-containing hepatocytes exhibit metabolic decompensation due to organelle distortion.
    • Hepatocyte death in ALD may be primarily mediated by neutrophilic attack rather than metabolic derangement alone.
