    Summary

    Bleomycin-induced pneumonitis (BIP) risk in testicular cancer patients can be predicted by monitoring creatinine clearance, vital capacity, and alveolar volume. Early detection allows for chemotherapy adaptation in at-risk individuals.

    Background:

    • Bleomycin is a chemotherapy agent used for testicular cancer.
    • Bleomycin can cause pulmonary toxicity, known as bleomycin-induced pneumonitis (BIP).
    • Predicting BIP is crucial for patient management during chemotherapy.

    Purpose of the Study:

    • To identify pulmonary function test parameters and creatinine clearance that predict BIP in patients with disseminated testicular nonseminomatous tumor.
    • To evaluate the utility of discriminant analysis in predicting BIP.

    Main Methods:

    • Pulmonary function tests (transfer factor for carbon monoxide, diffusing capacity of the alveolar-capillary membrane, pulmonary capillary blood volume, alveolar volume, vital capacity) and creatinine clearance were measured in 39 patients.
    • Discriminant analysis was used to identify predictors of BIP.

    • Eight out of 39 patients (20.5%) developed BIP.
    • A combination of low-normal creatinine clearance, decreased vital capacity, and decreased alveolar volume, without a decrease in pulmonary capillary blood volume, indicated an increased risk of BIP.

    • Monitoring specific pulmonary function parameters and creatinine clearance can help predict BIP risk.
    • Adapting bleomycin chemotherapy based on identified risk factors may be beneficial for patients.
