    Summary

    Massive ascites, a fluid buildup in the abdomen, was observed in a patient with pelvic inflammatory disease linked to an intrauterine device. Surgical removal of pelvic organs successfully resolved the ascites.

    Background:

    • Pelvic inflammatory disease (PID) is a serious infection of the female reproductive organs.
    • Intrauterine devices (IUDs) are a common form of contraception but can be associated with an increased risk of PID.
    • Ascites, the accumulation of fluid in the peritoneal cavity, can be a rare complication of various intra-abdominal processes.

    Observation:

    • A case of massive ascites was identified in a female patient.
    • The patient had a history of pelvic inflammatory disease (PID) associated with intrauterine device (IUD) use.

    Findings:

    • Exploratory laparotomy was performed to investigate the cause of massive ascites.
    • Surgical extirpation (removal) of the pelvic organs was undertaken.
    • Following the surgical procedure, the ascites resolved completely.

    Implications:

    • This case highlights a rare presentation of massive ascites secondary to IUD-related PID.
    • Surgical intervention, including pelvic organ extirpation, may be an effective treatment for refractory ascites in such cases.
    • Further research into the mechanisms linking IUDs, PID, and ascites is warranted to improve patient outcomes.
