    Summary

    Severe acne vulgaris flare-ups in adolescents can present with systemic symptoms. Oral steroids effectively managed severe acne exacerbations unresponsive to antibiotics.

    Background:

    • Acne vulgaris is a common pilosebaceous unit disease.
    • Severe inflammatory acne can rarely present with systemic manifestations.

    Observation:

    • A 17-year-old male experienced a sudden, severe exacerbation of acne vulgaris with extensive nodular, ulcerated lesions.
    • The patient presented with systemic symptoms including fever, arthralgia, weight loss, leukocytosis, and elevated ESR.
    • Conventional antibiotic treatments were ineffective in controlling the severe cutaneous and systemic symptoms.

    Findings:

    • Oral corticosteroids rapidly resolved the inflammatory lesions and systemic symptoms.
    • Attempts to taper steroid dosage resulted in relapses.
    • Alternative treatments with dapsone and clofazimine were unsuccessful.

    Implications:

    • This case highlights a rare, severe presentation of acne vulgaris requiring systemic immunosuppression.
    • Corticosteroids appear to be a crucial treatment modality for such severe, recalcitrant cases.
    • Further research into the pathogenesis and optimal management of severe acne flares with systemic involvement is warranted.
