    Many patients with major depressive episode (MDE) remain symptomatic for years, challenging the episodic definition of depression. Lack of adequate early treatment is linked to this chronicity in major depression.

    Background:

    • Major Depressive Disorder (MDD) is often characterized as episodic.
    • The Diagnostic and Statistical Manual of Mental Disorders, 3rd Edition (DSM-III) defines major depression as episodic with potential for relapse or recurrence.
    • Long-term symptom persistence challenges the episodic nature of MDD.

    Purpose of the Study:

    • To investigate the chronicity of major depressive episodes in a patient cohort.
    • To examine the compatibility of persistent depressive symptoms with the episodic definition of major depression.
    • To identify factors associated with the chronicity of major depression.

    Main Methods:

    • Prospective naturalistic study design.
    • Follow-up of 120 patients treated for a current major depressive episode.
    • Assessment of adherence to DSM-III criteria for major depressive episode throughout the follow-up period (1-4 years).
