    Background:

    • Diabetic glomerulosclerosis is a common complication of diabetes mellitus.
    • Superimposed glomerulonephritis can accelerate kidney function decline in diabetic patients.

    Purpose of the Study:

    • To investigate the immunopathologic findings in renal biopsies of patients with diabetic glomerulosclerosis and superimposed glomerulonephritis.
    • To understand the susceptibility of diabetic glomeruli to immune-mediated injury.

    Main Methods:

    • Analysis of renal biopsies from 18 patients with diabetic glomerulosclerosis.
    • Histopathological examination to identify diabetic glomerulosclerosis patterns (nodular vs. diffuse) and superimposed glomerulonephritis types.

    Main Results:

    • 10 patients had nodular and 8 had diffuse diabetic glomerulosclerosis.
    • Common superimposed glomerulonephritis included mesangiopathic glomerulonephritis (8 patients), focal proliferative glomerulonephritis (4 patients), and membranous glomerulonephritis (3 patients).
    • Rapid loss of renal function or atypical urinalysis suggested superimposed immune complex disease.

    Conclusions:

    • Diabetic renal disease patients are susceptible to superimposed immune complex diseases.
    • Altered glomerular structure and function in diabetes may predispose to immune-mediated injury.
    • Prompt identification of superimposed glomerulonephritis is crucial for managing diabetic kidney disease.
