Diagnosing hyperthyroidism in elderly patients is challenging. Standard thyroid function tests, including total thyroxine (T4) and triiodothyronine (T3), may appear normal, complicating diagnosis.

    Area of Science:

    • Endocrinology
    • Geriatric Medicine
    • Internal Medicine

    Background:

    • Hyperthyroidism diagnosis can be complicated in elderly patients.
    • Thyroid function tests may present atypical results in older individuals.

    Purpose of the Study:

    • To compare thyroid function test results in elderly and young hyperthyroid patients.
    • To evaluate the reliability of various thyroid tests for diagnosing hyperthyroidism in the elderly.

    Main Methods:

    • Thyroid function tests including T4, FTI, T3U, T3, and radioactive iodine uptake were performed.
    • Results from 35 elderly hyperthyroid patients were compared to 48 young hyperthyroid patients.

    Main Results:

    • Total T4 and FTI were normal in a significant percentage of both elderly and young hyperthyroid patients.

    Related Experiment Videos

  • T3 uptake was found to be a poor diagnostic test in both age groups.
  • Radioactive iodine uptake varied, being normal in 27% of elderly Graves' disease patients and 70% of elderly toxic nodular goiter patients.
  • Conclusions:

    • Diagnosing hyperthyroidism in the elderly is difficult due to non-specific test results.
    • No single thyroid function test is sufficient to rule out hyperthyroidism in older adults.
