    Summary

    Primary linitis plastica of the rectum is rare and difficult to diagnose preoperatively. Careful exclusion of secondary involvement and ensuring tumor-free margins are crucial for successful treatment.

    Background:

    • Linitis plastica is a rare form of gastric cancer that can also affect the rectum.
    • Primary rectal linitis plastica presents diagnostic challenges.

    Observation:

    • This study presents five cases of primary linitis plastica of the rectum.
    • Clinical, radiologic, and pathologic features were analyzed.
    • Preoperative diagnosis was found to be difficult.

    Findings:

    • Histologic features of primary and secondary linitis plastica are indistinguishable.
    • Secondary involvement of the rectum is twice as common as primary.

    Implications:

    • It is mandatory to exclude other primary sites of origin before diagnosing primary rectal linitis plastica.
    • Ensuring tumor-free proximal and distal margins during resection is essential.
