    Background:

    • Thoracic nervous system tumors are rare, often presenting in the posterior gutter.
    • These tumors can involve the sympathetic chain and intercostal nerves, posing surgical challenges.

    Purpose of the Study:

    • To analyze the surgical outcomes and characteristics of thoracic nervous system tumors.
    • To discuss the excision of specific tumor types, including those with extension into the intervertebral foramen.

    Main Methods:

    • Retrospective review of 86 patients with thoracic nervous system tumors operated between 1964 and the study period.
    • Classification of tumors by location, histology, and surgical approach.

    Main Results:

    • 95% of tumors were benign (59% schwannomas, 20% neurofibromas, 17% ganglioneuromas).
    • 80% of tumors were located in the posterior gutter, involving the sympathetic chain/intercostal nerves.

    Conclusions:

    • Malignant thoracic nervous system tumors have a poor prognosis with a high risk of recurrence.
