    Summary

    Human chorionic gonadotropin (hCG) subunits may aid in diagnosing parathyroid carcinoma. Elevated hCG levels can also indicate associated, potentially malignant, pancreatic islet cell tumors in patients with primary hyperparathyroidism.

    Background:

    • Primary hyperparathyroidism (1° HPT) is a condition affecting parathyroid gland function.
    • Human chorionic gonadotropin (hCG) subunits (alpha and beta) are typically associated with pregnancy but can be markers for certain cancers.

    Purpose of the Study:

    • To investigate the diagnostic utility of hCG subunit concentrations in patients with primary hyperparathyroidism.
    • To determine if hCG subunits can differentiate parathyroid carcinoma from benign 1° HPT.
    • To assess the association of hCG subunits with multiple endocrine neoplasia type I (MEN1) and associated tumors.

    Main Methods:

    • Plasma and serum samples from 70 patients with 1° HPT were analyzed for hCG alpha and hCG beta concentrations.
    • Tumor tissue extracts from patients with parathyroid carcinoma and benign 1° HPT were analyzed.

  • Selective venous catheterization was performed in some patients to analyze hCG subunits in parathyroid venous effluent.
  • Main Results:

    • Two of three patients with parathyroid carcinoma showed elevated hCG subunits, which decreased post-surgery.
    • Parathyroid carcinoma tumor extracts contained high hCG subunit concentrations, unlike benign 1° HPT tissues.
    • Elevated hCG alpha was observed in one patient with benign 1° HPT, but generally, benign cases showed no significant elevations.
    • In MEN1 patients, 6 of 8 with active islet cell tumors had high hCG alpha levels (and one had high hCG beta).

    Conclusions:

    • Determination of hCG subunits in patients with 1° HPT may assist in diagnosing parathyroid carcinoma.
    • hCG subunit measurement could be valuable for screening for associated, likely malignant, pancreatic islet cell disease in MEN1 patients.
    • hCG subunit analysis offers a potential diagnostic and screening tool in specific endocrine and oncological contexts.
