Severe Epstein-Barr virus (EBV) pulmonary involvement can cause fatal infectious mononucleosis (IM) pneumonia, even in immunocompetent individuals. This review highlights the critical need for accurate diagnosis and management of EBV-related respiratory complications.

    Area of Science:

    • Infectious Diseases
    • Pulmonology
    • Virology

    Background:

    • Epstein-Barr virus (EBV) commonly causes infectious mononucleosis (IM).
    • Pulmonary involvement in IM is rare but can be severe.
    • Differential diagnosis of respiratory infections is crucial.

    Observation:

    • Two cases of severe EBV pulmonary disease are presented.
    • Cases were confirmed by EBV-specific serology.
    • Other respiratory pathogens were excluded through cultures and serology.

    Findings:

    • One case resulted in fatal infectious mononucleosis with severe pneumonia.
    • The fatal case occurred in a patient without a known immunologic disorder.
    • A review of previously reported cases of pulmonary involvement in IM is included.

    Related Experiment Videos

    Implications:

    • Severe EBV pulmonary disease can occur in immunocompetent individuals.
    • Highlights the importance of considering EBV in severe pneumonia cases.
    • Emphasizes the need for prompt and accurate diagnosis of EBV-related respiratory complications.
