    Summary

Corynebacterium diphtheriae endocarditis is a severe illness with a high mortality rate. Bacteriological diagnosis is challenging due to its similarity to saprophytic diphtheroids.

    Area of Science:

    • Infectious Diseases
    • Cardiology
    • Microbiology

    Background:

    • Endocarditis is a serious infection affecting heart valves.
    • Corynebacterium diphtheriae, the causative agent of diphtheria, can rarely cause endocarditis.

    Observation:

    • A case of endocarditis caused by Corynebacterium diphtheriae is presented.
    • Bacteriological identification of the organism was challenging due to its resemblance to non-pathogenic diphtheroids.

    Findings:

    • The presented case, along with previous reports, highlights the extreme severity and high mortality associated with Corynebacterium diphtheriae endocarditis.
    • Despite clinical signs and cerebrospinal fluid (CSF) abnormalities suggestive of meningitis, definitive meningitic involvement was not confirmed.

    Implications:

    • This case underscores the importance of considering rare pathogens in severe endocarditis.
    • Improved diagnostic methods are needed for differentiating pathogenic and saprophytic Corynebacterium species in clinical settings.
    • The high mortality rate necessitates prompt recognition and aggressive management strategies for this rare but devastating infection.
