Preoperative chemotherapy for Wilms tumor

Background:

• Wilms tumor is a common pediatric kidney cancer.
• Some Wilms tumors are unresectable at initial diagnosis.

Purpose of the Study:

• To evaluate the efficacy of preoperative chemotherapy in facilitating the surgical resection of unresectable Wilms tumors in children.

Main Methods:

• Nineteen children with unresectable Wilms tumor received 2-4 doses of vincristine preoperatively.
• Tumor size reduction and resectability were assessed post-chemotherapy.

Main Results:

• Sixteen of 19 patients showed marked tumor size reduction.
• Sixteen tumors became completely resectable after chemotherapy; one required additional irradiation.
• Pathological changes were most significant in the undifferentiated interstitial stroma and nodular blastema.

Conclusions:

• Preoperative chemotherapy with vincristine can effectively reduce Wilms tumor size.
• This approach facilitates surgical management of unresectable Wilms tumors.
• The treatment was well-tolerated with no serious complications.
