    Summary

Primary anterior mediastinal seminomas show variable outcomes. Factors like age over 35 and specific symptoms indicate a higher risk of disease progression in these rare tumors.

    Area of Science:

    • Oncology
    • Thoracic Surgery
    • Pathology

    Background:

    • Anterior mediastinal seminomas are rare germ cell tumors.
    • Understanding their clinical behavior and prognostic factors is crucial for patient management.

    Purpose of the Study:

    • To review the Mayo Clinic experience with primary anterior mediastinal seminomas.
    • To identify factors associated with disease progression and outcomes.

    Main Methods:

    • Retrospective review of patients with primary anterior mediastinal seminomas.
    • Analysis of clinical, radiological, and pathological data.
    • Follow-up assessment of disease status and survival.

    Main Results:

    • Seventeen patients had pure anterior mediastinal seminoma; nine achieved no evidence of disease, and eight died.
    • Four patients with mixed germ cell tumors containing seminoma had two survivors and two deaths.
    • Factors associated with progression in pure seminoma included age >35, fever, superior vena cava syndrome, adenopathy, and hilar disease on imaging.

    Conclusions:

    • Primary anterior mediastinal seminomas have a significant risk of metastatic disease.
    • Certain clinical and radiological features may predict a poorer prognosis in pure anterior mediastinal seminomas.
