    This study investigated endocrine function in lipoatrophic diabetes (LD). Plasmapheresis improved hypertriglyceridemia but not endocrine dysfunction, and led to cardiovascular complications.

    Background:

    • Lipoatrophic diabetes (LD) is a rare condition characterized by generalized loss of adipose tissue and often associated with metabolic complications.
    • Endocrine dysfunction, particularly involving the hypothalamic-pituitary axis, is frequently observed in patients with LD.

    Observation:

    • A 24-year-old female with LD presented with hyperprolactinemia, elevated ACTH, abnormal dexamethasone suppression, and exaggerated TSH response to TRH.
    • Plasmapheresis initially reduced severe hypertriglyceridemia but did not resolve endocrine abnormalities.
    • Plasmapheresis was associated with amenorrhea, galactorrhea, hypertension, and retinopathy.

    Findings:

    • Despite plasmapheresis improving hypertriglyceridemia, central endocrine dysfunction (hyperprolactinemia, abnormal ACTH secretion) persisted.
