    Background:

    • RhD immunization is a significant concern in D-negative individuals exposed to RhD-positive blood products.
    • Understanding factors influencing RhD immunization is crucial for transfusion safety and preventing hemolytic disease of the newborn.

    Purpose of the Study:

    • To investigate the effect of a small, passively administered dose of anti-D antibody on RhD immunization in D-negative male volunteers.
    • To determine if concurrent antibody administration alters the kinetics or magnitude of anti-D antibody formation.

    Main Methods:

    • 13 D-negative male volunteers received an intravenous injection of 51Cr-labeled RhD-positive red blood cells and an intramuscular injection of anti-D antibody.
    • Red blood cell survival was assessed, and anti-D antibody formation was monitored over 5 months.
    • A second injection was administered after 5 months, and antibody responses were compared to a control group without passive antibody administration.

    Main Results:

    • Red blood cell survival was comparable between test and control groups.
    • While antibody concentrations and timing were similar, a higher proportion of subjects in the test group developed anti-D antibodies after the first and second injections compared to controls.
    • The increase in responder proportion was not statistically significant but suggested a facilitative effect.

    Conclusions:

    • Passively administered anti-D antibody did not augment RhD immunization in terms of accelerated antibody formation or increased concentration.
    • A small dose of anti-D antibody may facilitate primary RhD immunization, potentially increasing the proportion of individuals who respond immunologically.
    • These findings suggest a mechanism where low-dose antibody can prime the immune system for enhanced response to subsequent antigen exposure.
