    Background:

    • Chickenpox (varicella-zoster virus) can lead to hepatic and central nervous system complications.
    • Reye syndrome and encephalitis are serious, though uncommon, sequelae of chickenpox.

    Purpose of the Study:

    • To retrospectively assess the incidence and nature of hepatic and central nervous system involvement in chickenpox patients.
    • To explore the underlying pathogenetic mechanisms of these complications.

    Main Methods:

    • Retrospective chart review of chickenpox cases at a metropolitan medical center.
    • Analysis of biochemical and clinical data to identify liver and central nervous system abnormalities.

    Main Results:

    • Of 58 patients, 28 showed biochemical liver involvement without inflammation.
    • 18 patients had liver abnormalities with Reye syndrome; 12 had inflammatory encephalitis.
    • No cases of isolated inflammatory liver involvement were observed.

    Conclusions:

    • Hepatopathy and hepatoencephalopathy (Reye syndrome) in chickenpox likely result from a cytotoxic mechanism, not direct viral replication.
    • Post-infectious encephalitis appears to be immune-mediated tissue damage.
    • Viral replication in the skin may induce macromolecular dysfunction in distant organs (liver, brain).
