    Summary

    This study identified pancreatic tumors with glucagon cells in four diabetes mellitus patients. These tumors, or glucagonomas, presented in three distinct clinical subtypes, including malignant forms.

    Background:

    • Pancreatic neuroendocrine tumors (PNETs) are rare neoplasms.
    • Glucagonomas, a subtype of PNETs, arise from alpha cells and can cause distinct clinical syndromes.
    • Diabetes mellitus is a common metabolic disorder with complex etiologies.

    Observation:

    • Four patients with diabetes mellitus were found to have pancreatic tumors expressing glucagon.
    • Tumor analysis revealed alpha-cell granules in three cases and pancreatic polypeptide (PP) immunoreactive cells in two.
    • Malignancy was observed in two tumors; one presented with the glucagonoma syndrome, and the other was associated with Multiple Endocrine Neoplasia type I (MEN-I).

    Findings:

    • The presence of glucagon-producing cells in pancreatic tumors associated with diabetes mellitus.
    • Histological confirmation of alpha-cell granules and detection of pancreatic polypeptide cells in these tumors.
    • Identification of distinct clinical presentations, including malignant glucagonomas and association with MEN-I syndrome.

    Implications:

    • These findings highlight the diverse clinical manifestations of glucagonomas.
    • Understanding these subtypes is crucial for accurate diagnosis and management of patients with pancreatic tumors and diabetes.
