    Sympathetic uveitis after eye trauma and vitrectomy can initially mimic placoid pigment epitheliopathy. Histopathology revealed choroidal granulomas and Dalen-Fuchs nodules as the cause.

    • Sympathetic uveitis is a rare, bilateral inflammatory condition affecting the uvea.
    • It typically occurs after ocular trauma or surgery.
    • Pars plana vitrectomy is a common surgical procedure for various retinal conditions.

    • A case of sympathetic uveitis following trauma and pars plana vitrectomy presented with early retinal, retinal pigment epithelium, and choroidal lesions.
    • These posterior focal lesions resembled acute posterior multifocal placoid pigment epitheliopathy.
    • Similar lesions were observed on gross examination of the affected eye.

    Findings:

    • Histopathological examination identified focal choroidal granulomas and overlying Dalen-Fuchs nodules as the cause of these lesions.
    • Four additional patients with sympathetic ophthalmia after trauma and vitrectomy were also identified.
