    Background:

    • Acute polyneuritis, a neurological syndrome, can affect children.
    • Infectious mononucleosis is caused by the Epstein-Barr virus (EBV).
    • Cytomegalovirus (CMV) is a common virus that can cause various symptoms.

    Observation:

    • A 4-year-old boy experienced recurrent acute polyneuritis.
    • His sister had infectious mononucleosis with positive Monospot tests.
    • The boy showed elevated antibodies for EBV and persistent CMV antibodies in serum and CSF.

    Findings:

    • The patient presented with recurrent symmetrical neuropathy.
    • Elevated Epstein-Barr virus-viral capsid antibody (EBV-VCA) titers were observed.
    • Persistent cytomegalovirus (CMV) complement fixation antibodies were detected in serum and cerebrospinal fluid (CSF), with oligoclonal bands present in CSF.

    Implications:

    • This case suggests a potential link between CMV infection and recurrent polyneuritis in children.
    • Epstein-Barr virus reactivation may trigger latent CMV, leading to neurological relapse.
    • Further research is needed to confirm the causal relationship between CMV and recurrent polyneuritis.
